arch_cpu_finalize_init() implementation.

locking, arch/ia64: Reduce header dependencies by moving XTP bits into the new header

We want to remove the #include  from , but for this
we have to move the XTP bits into a separate header first (as these bits
rely on  definitions), and include them in the .c files that rely
on those APIs.

mm: introduce include/linux/pgtable.h

manipulation functions.

Start with moving asm-generic/pgtable.h to include/linux/pgtable.h and
make the latter include asm/pgtable.h.

As said in commit f2c2cbcc35d4 ("powerpc: Use pr_warn instead of

dma-mapping: fix filename references

After commit cf65a0f6f6ff ("dma-mapping: move all DMA mapping code to
kernel/dma") some of the files are referring to outdated information,
i.e. old file names of DMA mapping sources. Fix it here.

Note, the lines with "Glue code for..." have been removed completely.
