commit 2b6ae0962b421103feb41a80406732944b0665b3 upstream.

Avoid that the hardware path is shown twice in the kernel log, and clean
up the output of the version numbers to show up in the same order as
they are listed in the hardware database in the hardware.c file.
Additionally, optimize the memory footprint of the hardware database
and mark some code as init code.

commit cab56b51ec0e69128909cef4650e1907248d821b upstream.

Fix the output of /proc/iomem to show the real hardware device name
including the pa_pathname, e.g. "Merlin 160 Core Centronics [8:16:0]".
Up to now only the pa_pathname ("[8:16.0]") was shown.

Split out all the bits that are purely for dma_map_ops implementations
and related code into a new <linux/dma-map-ops.h> header so that they
don't get pulled into all the drivers. That also means the architecture
specific <asm/dma-mapping.h> is not pulled in by <linux/dma-mapping.h>
any more, which leads to a missing includes that were pulled in by the
x86 or arm versions in a few not overly portable drivers.

PA-RISC uses a global spinlock to protect pagetable updates in the TLB
fault handlers. When multiple cores are taking TLB faults simultaneously,
the cache line containing the spinlock becomes a bottleneck.

This patch embeds the spinlock in the top level page directory, so that
every process has its own lock. It improves performance by 30% when
doing parallel compilations.

At least on the N class systems, only one PxTLB inter processor
broadcast can be active at any one time on the Merced bus. If a Merced
bus is found, this patch serializes the TLB flushes with the
pa_tlb_flush_lock spinlock.
