It will be used outside arch/powerpc, make it clear its a
powerpc configuration item.

And we already have CONFIG_PPC_E500MC, so that will make
it more consistent.

Don't inherit headers "by chances" from asm/prom.h, asm/mpc52xx.h,
asm/pci.h etc...

Include the needed headers, and remove asm/prom.h when it was
needed exclusively for pulling necessary headers.

We have many functionnalities common to 40x and BOOKE, it leads to
many places with #if defined(CONFIG_BOOKE) || defined(CONFIG_40x).

We are going to add a few more with KUAP for booke/40x, so create
a new symbol which is defined when either BOOKE or 40x is defined.

'struct ppc_inst' is an internal representation of an instruction, but
in-memory instructions are and will remain a table of 'u32' forever.

Replace all 'struct ppc_inst *' used for locating an instruction in
memory by 'u32 *'. This removes a lot of undue casts to 'struct
ppc_inst *'.

It also helps locating ab-use of 'struct ppc_inst' dereference.

The check of the emergency context initialisation in
vmap_stack_overflow is buggy for the SMP case, as it
compares r1 with 0 while in the SMP case r1 is offseted
by the CPU id.

Instead of fixing it, just perform static initialisation
of the first emergency context.

There is no defconfig selecting CONFIG_E200, and no platform.

e200 is an earlier version of booke, a predecessor of e500,
with some particularities like an unified cache instead of both an
instruction cache and a data cache.

Remove it.
