Add SPDX license identifiers to all files which:

 - Have no license information of any form

 - Have MODULE_LICENCE("GPL*") inside which was used in the initial
   scan/conversion to ignore the file

These files fall under the project license, GPL v2 only. The resulting SPDX
license identifier is:

  GPL-2.0-only

<title>scsi: esp_scsi: move dma mapping into the core code</title>

Except for the mac_esp driver, which uses PIO or pseudo DMA, all drivers
share the same dma mapping calls.  Move the dma mapping into the core
code using the scsi_dma_map / scsi_dma_unmap helpers, with a special
identify mapping variant triggered off a new ESP_FLAG_NO_DMA_MAP flag
for mac_esp.

Remove usage of the legacy PCI DMA API.  To make this easier we also
store a struct device instead of pci_dev in the dev field of struct
esp.

On systems with shared interrupts the interrupt routine might
be called as soon as the interrupt is enabled.
As this might happen before pci_set_drvdata() is called the
system would crash.

The am53c974 is a re-implementation of the tmscsim driver,
and provides the same functionality.
So remove the tmscsim driver and make am53c974 an alias to tmscsim.

revisions, from 'Latch SCSI Phase' to 'Latch SCSI Phase, display
chip ID upon reset, and enable 24 bit addresses'.
