ipw->attr_memory and ipw->common_memory are assigned with the
return value of ioremap. ioremap may fail, but no checks
are enforced. The fix inserts the checks to avoid potential
NULL pointer dereferences.

It's good to have SPDX identifiers in all files to make it easier to
audit the kernel tree for correct licenses.

Update the drivers/tty files files with the correct SPDX license
identifier based on the license text in the file itself.  The SPDX
identifier is a legally binding shorthand, which can be used instead of
the full boiler plate text.

This work is based on a script and data from Thomas Gleixner, Philippe
Ombredanne, and Kate Stewart.

Fix up trivial conflicts in drivers/misc/pch_phub.c (same patch applied
twice, then changes to the same area in one branch)

As planned by Arnd Bergmann, this moves the ipwireless driver to the
drivers/tty/ directory as that's where it really belongs.
