minix: fix add link's wrong position calculation

Fix the add link method.  The oosition in the directory was calculated in
wrong way - it had the incorrect shift direction.

CRED: Wrap task credential accesses in the Minix filesystem

Wrap access to task credentials so that they can be separated more easily from
the task_struct during the introduction of COW creds.

Change most current->(|e|s|fs)[ug]id to current_(|e|s|fs)[ug]id().

Change some task->e?[ug]id to task_e?[ug]id().  In some places it makes more
sense to use RCU directly rather than a convenient wrapper; these will be
addressed by later patches.

SL*B: drop kmem cache argument from constructor

Kmem cache passed to constructor is only needed for constructors that are
themselves multiplexeres.  Nobody uses this "feature", nor does anybody uses
passed kmem cache in non-trivial way, so pass only pointer to object.

Non-trivial places are:
	arch/powerpc/mm/init_64.c
	arch/powerpc/mm/hugetlbpage.c

This is flag day, yes.

iget: stop the MINIX filesystem from using iget() and read_inode()

Stop the MINIX filesystem from using iget() and read_inode().  Replace
minix_read_inode() with minix_iget(), and call that instead of iget().
minix_iget() then uses iget_locked() directly and returns a proper error code
instead of an inode in the event of an error.

minix_fill_super() returns any error incurred when getting the root inode
instead of EINVAL.

limit minixfs printks on corrupted dir i_size

Essentially a corrupted minix dir inode reporting a very large
i_size will loop for a very long time in minix_readdir, minix_find_entry,
etc, because on EIO they just move on to try the next page.  This is
under the BKL, printk-storming as well.  This can lock up the machine
for a very long time.  Simply ratelimiting the printks gets things back
under control.  Make the message a bit more informative while we're here.

Slab API: remove useless ctor parameter and reorder parameters

Slab constructors currently have a flags parameter that is never used.  And
the order of the arguments is opposite to other slab functions.  The object
pointer is placed before the kmem_cache pointer.

Convert

        ctor(void *object, struct kmem_cache *s, unsigned long flags)

to

        ctor(struct kmem_cache *s, void *object)

throughout the kernel

mm: Remove slab destructors from kmem_cache_create().

Slab destructors were no longer supported after Christoph's
c59def9f222d44bb7e2f0a559f2906191a0862d7 change. They've been
BUGs for both slab and slub, and slob never supported them
either.

This rips out support for the dtor pointer from kmem_cache_create()
completely and fixes up every single callsite in the kernel (there were
about 224, not including the slab allocator definitions themselves,
or the documentation references).
