uapi: wrap compiler_types.h in an ifdef instead of the implicit strip

The uAPI stddef header includes compiler_types.h, a kernel-only
header, to make sure that kernel definitions of annotations
like __counted_by() take precedence.

There is a hack in scripts/headers_install.sh which strips includes
of compiler.h and compiler_types.h when installing uAPI headers.
While explicit handling makes sense for compiler.h, which is included
all over the uAPI, compiler_types.h is only included by stddef.h
(within the uAPI, obviously it's included in kernel code a lot).

Remove the stripping from scripts/headers_install.sh and wrap
the include of compiler_types.h in #ifdef __KERNEL__ instead.
This should be equivalent functionally, but is easier to understand
to a casual reader of the code. It also makes it easier to work
with kernel headers directly from under tools/

m68k: Avoid CONFIG_COLDFIRE switch in uapi header

We should not use any CONFIG switches in uapi headers since
these only work during kernel compilation. They are not defined
for userspace. Let's use the __mcoldfire__ switch from the
compiler here instead.

hexagon: Remove CONFIG_HEXAGON_ARCH_VERSION from uapi header

uapi headers should not expose CONFIG switches since they are not
available in userspace. Fix it in arch/hexagon/include/uapi/asm/user.h
by always defining the cs0 and cs1 entries instead of pad values.

hexagon: Remove unusable symbols from the ptrace.h uapi

Kernel-internal prototypes, references to current_thread_info()
and code hidden behind a CONFIG_HEXAGON_ARCH_VERSION switch are
certainly not usable in userspace, so this should not reside
in a uapi header. Move the code into an internal version of
ptrace.h instead.

x86: Remove the arch_calc_vm_prot_bits() macro from the UAPI

The arch_calc_vm_prot_bits() macro uses VM_PKEY_BIT0 etc. which are
not part of the UAPI, so the macro is completely useless for userspace.

It is also hidden behind the CONFIG_X86_INTEL_MEMORY_PROTECTION_KEYS
config switch which we shouldn't expose to userspace. Thus let's move
this macro into a new internal header instead.

arch/ia64/include: remove CONFIG_IA64_DEBUG_CMPXCHG from uapi header

CONFIG_* switches should not be exposed in uapi headers.  The macros that
are defined here are also only useful for the kernel code, so let's move
them to asm/cmpxchg.h instead.

The only two files that are using these macros are the headers
arch/ia64/include/asm/bitops.h and arch/ia64/include/asm/atomic.h and
these include asm/cmpxchg.h via asm/intrinsics.h, so this movement should
not cause any trouble.

Remove two dated config leak ignore entries from scripts/headers_install.sh:
arch/alpha/include/uapi/asm/setup.h does no more leak any CONFIG_* symbol to
user-space any more since commit 3cb8b1537f8a ("alpha: Move setup.h out of
uapi").
Same holds for include/uapi/asm-generic/fcntl.h since commit 306f7cc1e906
("uapi: always define F_GETLK64/F_SETLK64/F_SETLKW64 in fcntl.h").

It's unusable from userland - it uses elf_gregset_t, which is not
provided by exported headers.  glibc has it in sys/procfs.h, but
the same file defines struct elf_prstatus, so linux/elfcore.h can't
be included once sys/procfs.h has been pulled.  Same goes for uclibc
and dietlibc simply doesn't have elf_gregset_t defined anywhere.

IOW, no userland source is including that thing.
