Pull more block updates from Jens Axboe:
 "Mostly items that came in a bit late for the initial pull request,
  wanted to make sure they had the appropriate amount of linux-next soak
  before going upstream.

  Outside of stragglers, just generic fixes for either merge window
  items, or longer standing bugs"
