The UV code attempts to build a set of tables to allow it to do
bidirectional socket<=>node lookups.
But when nr_cpus is set to a smaller number than actually present, the
cpu_to_node() mapping information for unused CPUs is not available to
build_socket_tables(). This results in skipping some nodes or sockets
when creating the tables and leaving some -1's for later code to trip.
over, causing oopses.
The problem is that the socket<=>node lookups are created by doing a
loop over all CPUs, then looking up the CPU's APICID and socket. But
if a CPU is not present, there is no way to start this lookup.
Instead of looping over all CPUs, take CPUs out of the equation
entirely. Loop over all APICIDs which are mapped to a valid NUMA node.
Then just extract the socket-id from the APICID.
This avoid tripping over disabled CPUs.

Pull x86 apic updates from Dave Hansen:
"This includes a very thorough rework of the 'struct apic' handlers.
Quite a variety of them popped up over the years, especially in the
32-bit days when odd apics were much more in vogue.
The end result speaks for itself, which is a removal of a ton of code
and static calls to replace indirect calls.
If there's any breakage here, it's likely to be around the 32-bit
museum pieces that get light to no testing these days.
Summary:
- Rework apic callbacks, getting rid of unnecessary ones and
coalescing lots of silly duplicates.
- Use static_calls() instead of indirect calls for apic->foo()
- Tons of cleanups an crap removal along the way"

strscpy()
`strncpy` is deprecated for use on NUL-terminated destination strings [1].
A suitable replacement is `strscpy` [2] due to the fact that it
guarantees NUL-termination on its destination buffer argument which is
_not_ the case for `strncpy`!
In this case, it means we can drop the `...-1` from:
| strncpy(to, from, len-1);
as well as remove the comment mentioning NUL-termination as `strscpy`
implicitly grants us this behavior.
There should be no functional change as I don't believe the padding from
`strncpy` is needed here. If it turns out that the padding is necessary
we should use `strscpy_pad` as a direct replacement.

Every callsite hands in the same constants which is a pointless exercise
and cannot be optimized by the compiler due to the indirect calls.
Use the constants in the eoi() callbacks and remove the arguments.

There is really no point to have a callback which compares numbers.
Add a field which allows each APIC to store the maximum APIC ID supported
and fill it in for all APIC incarnations.
The next step will remove the callback.

default_setup_apic_routing() is a complete misnomer. On 64bit it does the
actual APIC probing and on 32bit it is used to force select the bigsmp APIC
and to emit a redundant message in the apic::setup_apic_routing() callback.
Rename the 64bit and 32bit function so they reflect what they are doing and
remove the useless APIC callback.

This is only used on 32bit and is a wrapper around
physid_set_mask_of_physid() in all 32bit APIC drivers.
Remove the callback and use physid_set_mask_of_physid() in the code
directly,

apic::init_apic_ldr() is only invoked when the APIC is initialized. So
there is really no point in having:
- Default empty callbacks all over the place
- Two implementations of the actual LDR init function where one is
just unreadable gunk but does exactly the same as the other.
Make the apic::init_apic_ldr() invocation conditional, remove the empty
callbacks and consolidate the two implementation into one.

native_smp_prepare_cpus()
uv_system_init()
uv_system_init_hub()
which is long after the per CPU areas have been set up.

present, but a count that includes any missing ids for blades not
present; in other words, the range from lowest to highest blade id.

Fix incorrect mask names and values in calc_mmioh_map() that caused it
to print wrong NASID information. And an unused blade position is not
an error condition, but will yield an invalid NASID value, so change
the invalid NASID message from an error to a debug message.

The UV5 platform synchronizes the TSCs among all chassis, and will not
proceed to OS boot without achieving synchronization. Previous UV
platforms provided a register indicating successful synchronization.
This is no longer available on UV5. On this platform TSC_ADJUST
should not be reset by the kernel.

BIOS now sets the x2apic enabled bit (and the ACPI table) for extended
APIC modes. Use that bit to indicate if extended mode is set.

bbbd2b51a2aa ("x86/platform/UV: Use new set memory block size function")
added a call to set the block size value that is needed by the kernel
to set the boundaries in the section list. This was done for UV Hubbed
systems but missed in the UV Hubless setup. Fix that mistake by adding
that same set call for hubless systems, which support the same NVRAMs
and Intel BIOS, thus the same problem occurs.

A test shows that the output contains a space:
# cat /proc/sgi_uv/archtype
NSGI4 U/UVX
Remove that embedded space by copying the "trimmed" buffer instead of the
untrimmed input character list. Use sizeof to remove size dependency on
copy out length. Increase output buffer size by one character just in case
BIOS sends an 8 character string for archtype.

Testing shows a problem in that UV5 hubless systems were not being
recognized. Add them to the list of OEM IDs checked.

Testing shows that trailing spaces caused problems with the OEM_ID and
the OEM_TABLE_ID. One being that the OEM_ID would not string compare
correctly. Another the OEM_ID and OEM_TABLE_ID would be concatenated
in the printout. Remove any trailing spaces.

Testing shows a problem in that the OEM_TABLE_ID was missing for
hubless systems. This is used to determine the APIC type (legacy or
extended). Add the OEM_TABLE_ID to the early hubless processing.

apic::irq_dest_mode is actually a boolean, but defined as u32 and named in
a way which does not explain what it means.
Make it a boolean and rename it to 'dest_mode_logical'

struct apic has two members which store information about the destination
mode: dest_logical and irq_dest_mode.
dest_logical contains a mask which was historically used to set the
destination mode in IPI messages. Over time the usage was reduced and the
logical/physical functions were seperated.
There are only a few places which still use 'dest_logical' but they can
use 'irq_dest_mode' instead.
irq_dest_mode is actually a boolean where 0 means physical destination mode
and 1 means logical destination mode. Of course the name does not reflect
the functionality. This will be cleaned up in a subsequent change.
Remove apic::dest_logical and fixup the remaining users.

The enum ioapic_irq_destination_types and the enumerated constants starting
with 'dest_' are gross misnomers because they describe the delivery mode.
Rename then enum and the constants so they actually make sense.

The UV x2apic is strictly using physical destination mode, but
apic::dest_logical is initialized with APIC_DEST_LOGICAL.
This does not matter much because UV does not use any of the generic
functions which use apic::dest_logical, but is still inconsistent.

The changes in the UV5 arch shrunk the NODE PRESENT table to just 2x64
entries (128 total) so are in to 64 bit MMRs instead of a depth of 64
bits in an array. Adjust references when counting up the nodes present.

When the UV BIOS starts the kernel it passes the UVsystab info struct to
the kernel which contains information elements more specific than ACPI,
and generally pertinent only to the MMRs. These are read only fields
so information is passed one way only. A new field starting with UV5 is
the UV architecture type so the ACPI OEM_ID field can be used for other
purposes going forward. The UV Arch Type selects the entirety of the
MMRs available, with their addresses and fields defined in uv_mmrs.h.

Update UV MMRs in uv_mmrs.h for UV5 based on Verilog output from the
UV Hub hardware design files. This is the next UV architecture with
a new class (UVY) being defined for 52 bit physical address masks.
Uses a bitmask for UV arch identification so a single test can cover
multiple versions. Includes other adjustments to match the uv_mmrs.h
file to keep from encountering compile errors. New UV5 functionality
is added in the patches that follow.
