Pull printk updates from Petr Mladek:

 - Herbert Xu made printk header file self-contained.

 - Andy Shevchenko and Sergey Senozhatsky cleaned up console->setup()
   error handling.

 - Andy Shevchenko did some cleanups (e.g. sparse warning) in vsprintf
   code.

 - Minor documentation updates.
